The Background
Patients in home care continuously collect and submit their vital values ​​with an app and medical measuring devices. Continuous submission of vital values ​​gives the clinicians a fair insight into the current symptom picture without increasing their workload.
The Levvel Platform automatically compares the patient's condition with the reference values ​​set by the clinician. In the event of signs of deterioration in the patient's condition, the health professionals are notified immediately. This allows for quick intervention and changes to the treatment, e.g. changing medication or calling for a consultation. Thus, well-treated patients avoid unnecessary doctor and outpatient visits and time is freed up for other patients in the clinic.
Increased Productivity & Centralized Monitoring
Reduced workload: Mobile app allows caregivers to accurately know which patients they need to visit, which measurements each patient needs to be taken. Most importantly as measurements are taken and submitted in the app data is immediately transferred to Levvel Health’s web portal and the caregivers system of record (Nexus).
Centralized Monitoring: The Clinician Portal allows the doctors to monitor all patient recordings as they come in from multiple different caregiving homes through one centralized location.

Patients at high risk are pinned at the top to make sure the caregivers are able to navigate to their detailed views easily.
Patient Details
In the patient details page we can see all the measurements that have been taken at the patients bedside by the caregivers.
Different measurement severity are easily visualized within the dashboard list drawing the attention of users when viewed at a glance.

Doctors are responsible for monitoring and acknowledging that after the measurements are recorded the patient is in no danger.
Patient Reference Values
Patient Thresholds:
Early detection of any life threatening conditions starts when a user is created in Levvel Health. Thresholds are setup for patients when they are admitted to ensure if any recordings abnormalities come in the caregiver and Doctors are alerted immediately.
Adjust patient's individual reference values:
If need be doctors can adjust the patient's reference values for each individual patient. This can be relevant if, for example, you are treating a patient who has habitual reference values that are either too low or too high.
